Recently involved in a MOTOR VEHICLE ACCIDENT (MVA)
Individuals involved in a MVA must consult their family physician or physiotherapist within 10 days of the accident, in order to qualify for direct billing from their insurance company. Patients will be required to complete an AB-1 form and sign an AB-2 form (to be completed by their physiotherapist). Minors must have a parent/guardian complete and sign the above mentioned forms. Once the MVA claim is accepted and is classified within protocol, patients may be entitled to 10 or 21 treatments (within 90 days of the accident), fully covered by their insurance company, subject to approval.
